This Privacy Policy describes how when and why I collect, use and share information when you attend an appointment, purchase a product from me ,contact me or otherwise use my services. This is to Comply with the General Data Protection Regulations (GDPR) 2018 Information I Collect.
To aid your treatment you will normally provide me with personal information ,such as your name, address,medical information and/or payment which I will store securely in a locked cabinet in a locked room to which I am the only person who has access. Your details may also be digitally password protected by myself Denise Clark.
Why I need Your Information and How I Use It.
I rely on a number of legal basis to collect, use and share your information, including;
. For the purposes of the provision of healthcare as needed to fulfil your podiatry assessment and treatment or to provide support;
. When you provided your affirmative consent, which you may revoke at any time;
. If it is necessary to comply with a legal obligation or court order such as a legal claim, or retaining information about your purchases required by Tax law;
Information sharing and disclosure.
I may share your information for a very limited reasons and in limited circumstances;
. Medical professionals. With your consent I will share information with medical professionals such as your GP or consultant to allow continuity of care.
. Service providers, I may engage trusted third parties to perform functions and provide services. If so, I will share your details but only to the extent permitted by law and with your consent.
. Compliance with laws. I may collect ,use, retain and share your information if I am legally required to do so.

I retain your personal information only for as long as necessary to provide you with a service as described in my Privacy Policy. However, I may also be required to retain your information to comply with legal and regulatory obligation. The retention of podiatry records is normally a minimum of 8years, after last appointment or 6years if you have purchased a product in line with tax legislation.
Transfer off Personal Data outside the EU
I may store and process your data through third party hosting services in the US and other jurisdictions. As a result, I may transfer your personal information to a jurisdiction with different data protection and government surveillance laws. When this process occurs outside of the EU, I rely on Privacy Shield and Secure Socket Layer (SSL) as the legal basis for transfer.

You have a number of rights with regards to your personal data. While some of these rights apply generally, certain rights apply only in limited circumstances.
. Access. You have the right to access and receive a copy of the personal data I hold by contacting me. Please be aware that there may be a fee associated with this.
.Change, restrict, delete. You also the right to change, restrict my use of or delete your personal data. In the case of health records these are normally exempt from change and deletion requests.
. Object. You can object to my processing of some of your data based on my legitimate interests. In such cases, I will delete your data unless I have compelling and legitimate grounds to continue using that data or if it needed for legal reasons.
